Submit a Grievance or an Appeal

  • you are reporting a grievance that has not been resolved to the customer's satisfaction
  • you are reporting a grievance from a customer that involves risk management issues
  • further research is necessary to respond to the customer
  • the customer has requested a formal reply to his/her concern
  • the customer has threatened legal or media action
  • you are aware of a grievance and are unsure which option to choose
  • the customer is disputing their cost share
