Entity Readmission Prediction Analysis

Description

Readmission following hospitalization is a costly and often preventable event. It has been estimated that readmissions within 30 days of discharge cost Medicare more than $17 billion dollars annually. A 2006 Commonwealth Fund report estimated that if national readmission rates were lowered to the levels achieved by the top-performing regions, Medicare would save $1.9 billion annually. This template captures common readmission metrics and dimensions which could be based on standard or customized rules for exclusions.
